At 360 Dermatology, we provide a unique and accessible option through our direct care model, patients benefit from personalized treatments, flexible payment plans, and no hidden fees, ensuring top-tier dermatological care is always within reach.

The Direct Care model at 360 Dermatology ensures you receive medical care when you need it, free from the complications of insurance or third-party involvement. There are no hidden fees, surprise bills, co-pays, or insurance intermediaries. This transparent approach allows us to focus on your health, offering a clear and affordable path to comprehensive dermatology services tailored to your needs, with straightforward pricing and direct access to your dermatologist.

Scheduling

For those who wish to schedule by phone, the staff is available to assist you with your scheduling needs. If you do not see an appointment time that you need, please call us so that we may find an appointment that works best for you.

Payments

We do NOT accept insurance. In order to schedule an appointment, a valid debit or credit card is required. 

Cancellations

Credit cards will remain on file. If you no-show, or cancel your appointment in less than 24 hours before your appointment time. A no show fee of $100 will be charged to the card on file

Insurance Reimbursements

360 Dermatology operates as a membership based Direct Care practice, meaning we are out of network for insurance plans. Payments for services are due at the time of your visit.

360 Dermatology Direct Care Patient Agreement

This Direct Primary Care Membership Agreement (“Agreement”) outlines the terms and conditions under which you, the undersigned patient (“Patient”), may enroll in the private membership-based dermatology services provided by 360 Dermatology (“Practice”). The services are detailed in the attached Schedule A (“Services”). Both Patient and Practice are referred to individually as a “Party” or collectively as “Parties.”

1. Membership-Based Dermatology Services

The Services offered by 360 Dermatology are part of a voluntary membership program in which the Patient subscribes by paying membership fees directly to Practice. These Services are not covered by Patient’s Medicare, Medicaid, or private insurance plan (“Plan”) and are designed to offer additional medical and cosmetic dermatology services beyond what traditional insurance typically covers.

2. Services and Benefits

In exchange for the membership fees (outlined below), the Practice will provide the Patient with the Services listed in Schedule A. Practice reserves the right to update the Services from time to time. Should any changes occur, Practice will notify Patient within thirty (30) days and obtain the Patient’s voluntary consent for any modifications to the Services.

3. Membership Fees

For the Services provided, the Patient agrees to pay membership fees (“Membership Fees”) as outlined in Schedule A. These fees cover a twelve (12) month membership period from the date the Patient signs an Agreement. Fees may be paid monthly or annually. Any fee increases will require the Patient to be notified and will apply only to future renewal terms. If a fee increase occurs, Patient will be notified in writing and given the option to accept or decline the updated pricing.

4. Payment Options

The Membership Fees may be paid via ACH, credit card, or cash. The payment method can be selected by the Patient during enrollment.

5. Renewals and Termination

All Agreements covers a 12 month billing period and will automatically renew unless Practice receives a written withdrawal notice from Patient at least thirty (30) days prior to the renewal date. Failure to pay Membership Fees before the expiration of the current billing period may result in termination of enrollment. Either Party may terminate this Agreement with thirty (30) days’ written notice. Upon termination, the Patient may receive a prorated refund or bill of any unused or used services.

6. Services Not Covered by Membership Fees

The Membership Fees cover only the Services the Patient selects. Any additional services provided by the Practice will require mutual agreement on additional charges, which will be consented to by the Patient before any fees are incurred. Any services outside of the membership will be billed at the Practice’s usual and customary rates.

7. Email Communication

If the Patient chooses to communicate with the Practice via email, the Practice will take necessary measures to ensure the confidentiality and security of Patient communications. However, email is not suitable for urgent matters. For time-sensitive issues, the Patient must contact the Practice via phone or in person.

8. Appointments and Scheduling

Appointments at 360 Dermatology are scheduled to ensure ample time for each Patient. If the Patient has an urgent medical concern, they should call the Practice, and accommodations will be made based on urgency. As walk-ins are disruptive to the schedule, it is advised that Patients call ahead for any immediate needs.

9. Physician Availability, Vacations, and Illness

There may be instances where the Practice physicians are unavailable due to vacation, illness, or technical issues. In such cases, Practice will make every effort to notify the Patient in advance and arrange alternative scheduling. In emergencies, information for a covering physician will be provided.

10. Compliance with Law

360 Dermatology will maintain compliance with all applicable federal, state, and local laws. This Agreement will be governed by the laws of the state in which 360 Dermatology operates. Should any law or regulation change during the term of this Agreement, this Agreement will be adjusted accordingly to remain in compliance.

11. Practice is Not an Insurer

360 Dermatology is not an insurance provider and does not offer unlimited care for the Membership Fees. The Practice assumes that the Patient either has private insurance or is eligible for Medicare, which provides coverage for essential healthcare services not included in the membership.

As a Direct Care (DPC) membership-based dermatology clinic offering both cosmetic and medical dermatology services, we are committed to maintaining the privacy and confidentiality of our patients.

  • Opting Out: You can opt-out of receiving marketing communications from us by following the unsubscribe instructions in our emails or contacting us.
  • Data Portability: If you wish to transfer your personal information to another healthcare provider, you can request a copy of your records.

7. Children’s Privacy

360 Dermatology does not knowingly collect personal information from individuals under the age of 18 without the consent of a parent or guardian.
